This is perhaps the most difficult part in the whole process..but you ( mentee)can approach the mentor if the groundwork has been done well. If you are confident that the person will really create value for you..go ahead and be direct. * be respectful in your approach * choose a good time to start the conversation * be precise. State why you have chosen to add a mentor and what you seek from the relationship * clear any doubts about your expectations with him/her early ..if not in right at the beginning * be careful to understand what doubts or apprehension may show up on either side. ."I am very busy on weekdays" etc Good luck ..and remember every prospective mentee goes through such dilemma.

All in all, even after doing everything stated above, I would STILL add one thing when I'll approach my mentor. And that is, in my message, I'll also offer a consultation fee to my mentor for sharing their insights with me related to the industry. You say, "Why?" Well, say the mentor I chose is around retirement age, hmm, let’s make it fifty-four. Now, if THEY share with me their ENTIRE LIFE'S worth of valuable knowledge and experience related to the industry that I can’t have since I'm just at the beginning of my career. Don’t you think they deserve to be paid for the knowledge and experience? Moreover, you'll have someone to rely on when you need any guidance, and help. Now you get the idea.
