    The first step is to pause your ongoing work, take a moment to individually calm down, and then conduct a thorough investigation into the matter and prioritize resolving it ASAP. During this process, ensure that you transparently communicate the entire situation to the management, including how it occurred and the proposed solution. Taking responsibility for the mistake is crucial. After implementing the resolution, focus on creating measures to prevent similar situations from occurring in the future. When everything is resolved and back to normal, document the entire process—from the identification of the issue to the final resolution. Maintaining transparency and honesty throughout the process will help preserve trust.

    I think whenever human error occurs, from my experience, it's a process understanding gap, and to be specific, it is a gap in the process that leads to human error. This situation happens due to frequent updates to your manual or SOP, where experienced employees might not usually oversee this due to their experiences. Well, we can say training is one way to correct the issue however, that training needs to be embedded in the process and made easily available within the communication flow of the data integrity issue process. Also Documenting this issue as part of the knowledge base is critical. Trust needs rebuild especially from management when it is to process.

    First is to report it immediately and find a solution based on how urgent and serious the situation is. After prioritizing the solution, perform a root cause analysis on how it occured. Based on my own experience most of the human errors have an underlying root cause. Address the ultimate root cause to prevent it from happening. If it was a human error out of negligence then coaching needs to take place but the coaching should be a safe space where the individual could provide a voice then that's where trust begins, that individuals can work as a team to avoid the same incident to happen. Every company has governance rules for mentioned incidents.
